USA Pickleball has updated the Visual Acuity Form that is available on the Referee Resources page through Spot80.  The form has changed to be more current and to reflect new email addresses where it needs to be returned.  Remember this still needs to be signed by the provider and the referee before submitting.  Many forget to sign this.

The current form will be accepted until September 1, 2026 at which time this new form will be the only official form used by USA Pickleball.
